We are excited to announce a special mobile passport service event hosted by Mike Foley’s office at the Montgomery County Clerk of Courts! Join us on Tuesday, June 18th, from 6:00 PM to 9:00 PM at the Huber Heights Library, located at 6243 Brandt Pike, Huber Heights, OH. This event offers a convenient opportunity to apply for your passport and start planning your travel adventures.

Can’t Make This Date? No Worries!

If you can’t attend the event on June 18th, don’t worry! You can still take advantage of passport services at several convenient locations across Montgomery County.

Regular Passport Service Locations and Hours

1. Downtown Dayton
   • Address: 451 W Third Street, Dayton, Ohio 45422
   • Hours: Monday – Friday, 8:00 AM – 3:30 PM
2. Moraine

What You Need for Passport Application

Regular Service (6-8 Weeks)

For a Passport Book (Land, Air, and Sea Travel):

1. Certified Birth Certificate, Naturalization Certificate, U.S. Consular Report of Birth Abroad, or Expired Passport
2. Government Issued Identification
3. Check or Money Order payable to the U.S. Department of State:
   • Adult (16 & Older): $130.00
   • Minor (15 & under): $100.00
4. A Passport Picture (Optional: available for an additional $15.00 fee)
5. $35.00 Clerk Acceptance Fee

For a Passport Card (Land & Sea Travel):

1. Same documents as above
2. Check or Money Order payable to the U.S. Department of State:
   • Adult (16 & Older): $30.00
   • Minor (15 & under): $15.00
3. $0.50 Citizenship Copy Fee
4. $35.00 Clerk Acceptance Fee

Expedited Service (2-3 Weeks)

For a Passport Book:

Special Notes for Minor Applicants

For minor applicants (ages 15 and under), both parents must be present unless one parent has sole custody. If one parent cannot be present, a DS-3053 (Minor Consent Form) must be notarized and submitted along with a copy of the non-applying parent’s photo ID. In cases where the non-applying parent cannot be located, a Special Family Circumstances form (DS-5525) may be used.

For applicants aged 16 and 17, one parent or guardian must be present.

Additional Services

• Passport Photos: Available for an additional fee of $15.00.
• Express Mail Fee: $29.32 for expedited service.
